模型1：六气皆从火化

一句话：风、热、湿、火、燥、寒六气为病，多从火化。 证据：

  • 《玄机原病式》热类：吐酸者，胃膈热甚则郁滞于气，物不化而为酸，酸者肝木之味。或言吐酸为寒者，误也。（《素问玄机原病式》热类）
  • 《保命集》伤寒论：天以常火，人以常动，动则属阳……故不可峻用辛温大热之剂。（《素问病机气宜保命集》伤寒论第六）
  • 热类分述"喘""呕""吐酸"皆归热——火气甚为夏热，衰为冬寒，病热则气甚而息粗。 应用：遇到貌似寒证（吐酸、下利、战栗）时，用此镜片先问"是否火化所现"。 局限：并非所有病皆火化——寒类确有真寒（诸病水液澄彻清冷皆属于寒），须脉证别之。

模型2：亢则害承乃制——兼化是虚象不可误治

一句话：本气过极反兼胜己之化，兼化是造化自抑的虚象，不是真实病气，治本气不治兼化。 证据：

  • 《玄机原病式》风类：风木为病，反见燥金之化，由亢则害，承乃制也。故诸风甚者，皆兼于燥。
  • 《玄机原病式》湿类：湿过极，则反兼风化制之。然兼化者虚象，而实非风也。
  • 《玄机原病式》寒类"癥"分述：其为治者，但当泻其过甚之气，以为病本，不可反误治其兼化也。 应用：见到"看似某气实是另一气"的矛盾象（风甚见燥、湿甚见风、热甚见寒战），用此镜片判断是兼化假象，治本气。 局限：兼化与真实兼病需别——若真有两气相兼，不可一概归兼化。

模型3：审机求属——治病必求其本

一句话：察病机之要理，施品味之性用，然后明病之本焉。故治病不求其本，无以去深藏之大患。 证据：

  • 《保命集》病机论第七：察病机之要理，施品味之性用，然后明病之本焉。故治病不求其本，无以去深藏之大患。
  • 《保命集》病机论：谨守病机，各司其属，有者求之，无者求之，盛者责之，虚者责之，必先五胜，疏其血气，令得调达而致和平。 应用：不被症状表面牵着走，求其所属病机（风属肝、热属心、湿属脾、燥属肺、寒属肾），再定治法。 局限：求属须脉证合参，不可仅凭一症定属。

决策启发式

  1. 见火热证先从火化推

    • 应用：见喘、呕、吐酸、暴注、谵妄、疮疡，先问"是否火化"
    • 案例：吐酸世俗作寒治用温药，我辨为"火盛制金不能平木，肝木自甚故为酸"，宜寒凉（《玄机原病式》热类）

  2. 兼化假象不误治本气

    • 应用：风甚兼燥（筋缩劲切）不可作燥治，湿甚兼风（痉强直）不可作风治
    • 案例：湿过极反兼风化，"兼化者虚象，而实非风也"（《玄机原病式》湿类）

  3. 不可峻用辛温大热

    • 应用：时代不同运气有变，天以常火人以常动，纵获一效其祸数作
    • 案例：辛温攻表不中，病转甚发惊狂衄血斑出，皆热药所致（《保命集》伤寒论）

  4. 瘕病亦有热者须脉证别之

    • 应用：癥瘕俗多归寒，但阳气郁结怫热壅滞亦可坚硬不消
    • 案例：《玄机原病式》寒类瘕分述——非寒癥瘕者宜以脉证别之

  5. 用热远热用寒远寒

    • 应用：司气同者不可犯，司热之年不用热药，司寒之年不用寒药
    • 案例：《保命集》气宜论——发表不远热，攻里不远寒，但不发不攻而犯寒犯热则病益甚
